TimvdeAfter potential exposure, after how long can I reasonably assume that I'm not infected if I do a negative self-test? (disregarding the accuracy of the tests, it's on a "best effort" basis, I don't even *need* to isolate and do a test according to the official guidelines)12:11
TuvixIf we're talking a Rapid Antigen Test (a "RAT") timing is a factor. If you test too soon after the possible exposure, the virus may not have reproduced enough to be picked up on the test, which is far less sensitive than a PCR test.12:13
TimvdeTuvix: Yes, an antigen test indeed. I know that timing is a factor, that's why I ask :D12:14
TuvixYou probably wouldn't expect a RAT to show positive even if you did pick up the virus, and I'd wait at least 2 days, and then ideally do 2 tests at least 24-hours (1 day) apart from each other.12:14
TimvdeOh, that's even shorter than I expected. I was not sure if 4 days would be enough or if I'd better wait for 5 days.12:15
TuvixThe day-spread helps both possibly catch a variation on the timing, and improves the odds about a false-negative, which can be fairly substantial. The other option too is to get a PCR test; many places do have those available for free with scheduling at various accessible clinics & pharmacies.12:15
TuvixWell, I say no earlier than a couple days, but 3-5 may be even better; that'll probably be closer to the "peak" of disease replication.12:15
TuvixBut doing 2 tests a day or 2 apart is still the best approach. While not foolproof given the error rate, it's better than nothing. I'd keep an eye on how you're feeling too, as even mild symptoms might suggest going for a more sensitive test.12:16
TimvdePCR tests are only available here if you have symptoms (and I would go for PCR in that case)12:16
TuvixAh, gotcha. Let me see if I had any more specific studies on the timing. I seem to recall bookmarking one, but it may take a moment to dig out depending on how well I tagged it :P12:19
Tuvix"individuals with daily testing during an omicron outbreak in December 2021 showed a median of two days of apparently false-negative RAT results with four recorded transmission events during the period preceding the positive PCR result"12:21
TimvdeNo problem :) I had possible exposure on Friday evening, and I'm considering going to work tomorrow if I test negative in the morning. So let's say that's 3.5 days. We obviously still have basic precautions at work (proper ventilation, distance, and masks when distance cannot be kept).12:21
TuvixSo, I'd do it no *earlier* than 2 days, but that's just the median of days of RAT false-negative; better money might be waiting somewhere between 3 to 5 days, and then doing 2 tests at least a day (or even 2) apart.12:21
TuvixHere's the reference that's quoted from: https://link.springer.com/article/10.1007/s00430-022-00730-z12:22
TimvdeThanks! Let's have a look :)12:22
TimvdeTuvix: it doesn't say if the RAT was still negative after the positive PCR test though :P12:23
TuvixRight, no error bars on that statement, but there may be more data in the full study, I'm still reading the other portions of that paper.12:24
TimvdeI also go way deeper in my nose with the swab than the instructions say :P12:25
TimvdeAnd I also swab my throat (but not very extensively because of my gag reflex)12:25
TuvixNot all the test types are equal either; some products are better or worse by comparison, but I'm not currently aware of a great reference for that. If I remember right,t his paper does talk briefly about the various test types though and their quality.12:26
TimvdeHere's a comparative study: https://www.eurosurveillance.org/content/10.2807/1560-7917.ES.2021.26.44.210044112:27
TuvixThe headline figure is that among all PCR-positive cases at a walk-up testing center, 65.2% of all (not just high-load) positive cases actually showed positive on the BinaxNOW™ RAT12:27
TimvdeThe tests I buy are New Gene, which are among the better ones according to that study12:27
TuvixHmm, does the one you linked mention which varient that's tested against? Be aware Omicron has a worse performance on the tests than prior VoCs12:28
TimvdeI don't think so, but it was definitely pre-omicron.12:29
TuvixThe comparison may still be valid to loosely rank the various tests, but Omicron has been a real game changer in areas like this.12:29
TimvdeI'm aware, loosely ranking the tests is the best I can do, I'm afraid :P12:29
Timvde(with the data I have, that is)12:30
TuvixRight, you've got to work with what you have, and Omicron has certainly made a mess of some of this…12:30
TuvixIt's good that you're keeping aware of some of the issues though; it's easy to look for quick answers, but often science doesn't give us the quick or exact answers we want.12:31
TuvixOne of my favourite comics on that concept: https://xkcd.com/68312:31
TimvdeI also just found this list, but it doesn't mention anything omicron-specific for New Gene: https://ec.europa.eu/health/system/files/2022-02/covid-19_rat_common-list_en.pdf12:33
TuvixAh, the study I cited is also BA.1 specific, so BA.2 "might be" different yet (my words, not the study's) but it's probably closer to BA.1 than to Delta for instance.12:33
BrainstormNew from Reddit (test): CoronavirusUK: Covid in Scotland: Vaccine passports and classroom masks rules end → https://old.reddit.com/r/CoronavirusUK/comments/t3cwjy/covid_in_scotland_vaccine_passports_and_classroom/12:34
TuvixOh man, some of these tests (yours isn't in the study I cited either, so it' snot directly relevant), 5 of them, were less sensitive requiring 8-times more replicating virus to test positive, so there's a good variation of quality between RATs.12:36
TuvixAt least your find would suggest you have one of the better brands, at least if the pre-Omicron figures hold for BA.1 & BA.2 still.12:36
TimvdeYes, which is why I already looked for a comparative study in the past :) There are also tests for sale here that are embarrassingly bad.12:37
TuvixI think many people really don't care and "just want their negative result" so they can go back to whatever they were doing.12:37
TuvixI've even heard of some places that have QR codes in tests where people just open the package and discard the still-unused test just to get the code :\12:38
TimvdeYea, but I do care :P There's no need for testing here (according to the guidelines), so I could just not do one. If I do it, I rather do it properly.12:39
Timvde(hence also why I swab my throat and go like 5 cm inside of my nose)12:40
TuvixHmm, the study I cited also seems to hint that pre-Omicron results aren't really useful in terms of Omicron :(12:40
Tuvix"In summary, these results indicate that the analytical sensitivity of RATs for omicron-containing respir- atory samples cannot be deduced from previous evaluations of other SARS-CoV-2 variants." (p. 8 of the PDF version)12:40
TimvdeOh, that's bad news :(12:40
TuvixIndeed, these varients are quite annoying if I may say so…12:40
TimvdeDoes it have a list of brands somewhere? If it indicates a particular test as working well for omicron and it's for sale here, I can also get that one.12:42
TuvixYea, 2nd paragraph on p. 412:43
TimvdeHmm, they don't ring a bell. I have found New Gene, Siemens, AllTest (the really bad one) and FlowFlew here, but none of those are in the paper.12:46

Tuvix"among the approximately 580 RATs [41] currently available on the German market not even half has been evaluated by either the Paul-Ehrlich-Institute or independent diagnostic labo- ratories for any SARS-CoV-2 variant, let alone omicron."13:10
Tuvix"experimental studies have estimated that around 1000 virus particles may be suf- ficient for infection of a new host […] loads required for a positive score in SARS-CoV-2 RATs range more than 1.000-fold higher."13:12
TuvixNow, I'm not sure how long it takes to produce that 1000x viral load (since infection might multiply faster as more virus exists) but that's a sense of scale of infection vs. test-sensitivity.13:13
TuvixI mean, presumably a median of 2 days going by the report of course :P but my point was more than "1000x" is about the test's sensitivity, not the time that'll take.13:13
TuvixAlso interseting, "Preliminary reports indi- cate that COVID-19 patients with omicron may shed even less virus than individuals infected with previous VoCs" <-- looks like Omicron spreads with less virus, yet still can infect much more easily. Sneaky bastard…13:14
Timvde1000 may be enough for infection, but it's probably not enough for being infectuous, which is the main driver for me to do these tests13:14
TuvixYea, that's kind of a lower-bound for infection potential, but it gives a broad sense of scale.13:15
TuvixThings in the real world don't work the same as a lab of course, and a lot depends on circumstance; where are the people at risk, what mitigation do they have, what is the quality of environmental mitigations, etc.13:16

TimvdeOf course :) I'm just trying to do my best here with the information I have, and without keeping myself locked up. By trying to avoid high-risk situations and doing a lot of self-tests, I hope to find a balance.13:25
TuvixBlah, the CDC's "new" county risk tool is awful; it no longer shows the whole country, making you pick *both* a state and county before it will return the risk assessment, which seems to largely just consider lagging indicators like hospitals.13:45
TuvixOh, no, it does still have a national view, it just paints a much more rosey pitcture of things in places I wouldn't expect based on continued hospital & death counts. Could just be a shift into the more-optimistic outlook too and hoping we don't see another surge. ie: "wishful thinking"13:47
